SAN FRANCISCO (AP) ? Plans to use an array of powerful air cannons in an undersea seismic study near a Central Californianuclear power plant have federal and state officials juggling concerns over marine life with public safety.
Pacific Gas & Electric Co. wants to use big air guns to emit strong sound waves into a large, near-shore area that includes parts of marine reserves to make three-dimensional maps of fault zones, some of which were discovered in 2008, near its Diablo Canyon nuclear power plant.
But a state study, mandated by AB1632, signed into law in 2006, found the project is likely to have "unavoidable adverse effects" on marine life and the environment. Biologists, environmental groups and fishermen have opposed using the high-energy air guns, saying the blasts have potential to harm endangered whales, California sea otters and other creatures frequenting these waters.
"I am very concerned about impacts to marine mammals, especially some of the large whales including blue, fin, and humpback whales," said John Calambokidis, an Olympia, Wash.-based marine biologist who has studied Pacific Ocean whales for decades. "There are many uncertainties on the impact of this type of operation on whales, especially since we have not seen this type of large air gun survey off California for a long time."
The $64 million, ratepayer-funded effort to understand seismic threats to the plant has intensified since the disastrous 2011 Tohoku quake and tsunami, which disabled reactors at Japan's Fukushima Dai-ichi nuclear power plant. Quake experts were surprised by the 9.0-magnitude quake on a fault that scientists did not believe would produce a quake stronger than 8.0.
Although the Japan disaster demonstrated that predicting the strength of a quake on a given fault is an inexact science, PG&E wants to know if the newly discovered faults near San Luis Obispo are connected to existing ones that have already been studied. Seismologists typically use a fault's length to estimate the maximum possible earthquake it can produce.
"People need to understand, we're living in the world post-Fukushima, so we need to go back and review everything we think we know about the seismic threat situation around important structures like this power plant," said Bruce Gibson, a former seismologist who now serves as a San Luis Obispo County supervisor.
"Unfortunately, from an environmental impact standpoint, the only real way to get the images is to put high energy sound into the earth."
If the project gains approval from myriad agencies, scientists would tow up to 18 air guns behind a boat and blast loud sound into the water over a 530-square-nautical-mile area. Hundreds of sensors would be placed strategically on the seafloor, picking up the reverberations and allowing computers to create three-dimensional maps in technology similar to an ultrasound.
The air guns and sensors would be dragged through an area that includes two state marine protected areas ? Cambria and White Rock ? and is adjacent to the Monterey Bay National Marine Sanctuary. Dozens of endangered and threatened species use these waters.
A similar seismic survey is being planned near the state's other nuclear plant at San Onofre, in San Diego County to the south.
A State Lands Commission environmental impact study found on Aug. 20 there would be "unavoidable impacts" to marine life in the area during the San Luis Obispo testing.
But the commission also concluded the "benefit of the project outweighs the unavoidable adverse impacts," said Jennifer DeLeon, a senior environmental scientist at the commission.
While similar high energy seismic surveys have been done on the Pacific Coast ? most recently off Washington ? PG&E said monitors there did not observe harm to whales or other marine mammals.
The powerful cannons used in these projects can be fatal to animals that stray too close to them. Also, biologists said the loud noises could drive migrating whales and their calves apart, and that mortally wounded whales often sink in the ocean, so it is difficult to see how the tests affect the creatures.
Efforts to mitigate such impacts will reduce, but not eliminate, harm to animals, according to the company and earth scientists.
"The sound source for the PG&E imaging project is a type that has been used for several decades by scientists and industry," Donna Blackman, a geophysicist at Scripps Institution of Oceanography in San Diego, said in an email. "Known cases of possible impacts on marine mammals are very few. An integral part of using this type of system is to have continuous monitoring for whales within close range of the ship."
PG&E said it is spending $8 million on monitoring for the project, said Mark Krausse, a PG&E director.
"If the ship is coming within 1.1 mile of any mammal, not just a marine or listed, but any mammal, we have to shut down," Krausse told the California Fish and Game Commission on Monday.
PG&E wanted to start work Nov. 1 and continue through Dec. 31 ? a time window believed to have lower whale traffic off the Central Coast. But the company has asked for an extension of its hearing before the California Coastal Commission, and other agencies are not expected to approve testing permits by then.
Major environmental groups such as the Sierra Club and Natural Resources Defense Council oppose the plan, saying adequate seismic research already has been done, and that too little is understood about potential long-term impacts of the air guns on the marine environment.
"The marine protected areas were created (so) marine wildlife could thrive without human interference," said Amanda Wallner of Sierra Club California. "We share concern over earthquake risk at Diablo Canyon. However, we don't believe this is the best way or the only way to determine seismic risks."
Julio M. Pow-Sang, M.D., chair of Moffitt Cancer Center's Department of Genitourinary Oncology, and colleagues have published two prostate cancer articles in the September issue of JNCCN ? The Journal of the National Comprehensive Cancer Network. The articles review and clarify recent updates made to the National Comprehensive Cancer Network's (NCCN) guidelines for the screening and treatment of prostate cancer.
According to the NCCN, nearly 242,000 cases of prostate cancer will be diagnosed in 2012, accounting for 29 percent of new cancers among men. It is estimated that more than 28,000 men will die from the disease this year.
According to the journal, prostate cancer is "over diagnosed and over treated" and is subject to a controversy "fueled by large screening studies." This month, Prostate Cancer Awareness Month, the NCCN has offered support for active surveillance, a strategy by which men with low-risk tumors, often classified as clinically insignificant, are monitored over time.
"Depending on the disease characteristics and the patient's life expectancy and personal preference, active surveillance may be a viable alternative to immediate and radical treatment," Pow-Sang said. "For others with high-risk, localized tumors or locally advanced disease, there are recent treatment advances that should be considered."
New agents and therapy combinations
According to the NCCN, external beam radiation and androgen deprivation therapy have been the traditional treatments for patients with high-risk tumors. However, the NCCN reports that brachytherapy, either permanent seed implant or temporary placement (high dose radiation), combined with external beam radiation and androgen deprivation therapy is increasingly being used for these patients.
The first article, Prostate Cancer, version 3.2012, noted that two new agents ? abiraterone acetate and immunotherapy sipuleucel-T ? are welcome therapeutic additions for patients with late-stage disease and poor prognosis.
Abiraterone acetate, which inhibits a key enzyme related to prostate cancer, can be given to patients after their chemotherapy. Studies have shown that it can also be used in chemotherapy-na?ve patients.
Sipuleucel-T, a second line therapy, is a live cancer vaccine approved in 2010 for treating asymptomatic or minimally symptomatic patients with metastatic prostate cancer. The NCCN has recommended the new drug for use after chemotherapy failure or as treatment along with chemotherapy.
"The new NCCN guidelines highlight important updates to the management of prostate cancer," concluded Pow-Sang and his co-authors. "These protocols are updated annually and more often when new, high-quality clinical data become available. The guidelines are based on evidence from clinical trials combined with expert consensus."
Active surveillance and PSA density, percent free, and biopsy cores
In a second article, Enrollment Criteria Controversies for Active Surveillance and Triggers for Conversion to Treatment in Prostate Cancer, Pow-Sang and co-author David D. Buethe, M.D., a urologic oncology fellow at Moffitt, examine the controversy and criteria for implementing active surveillance for patients with asymptomatic prostate cancer and identify the triggers that would convert active surveillance patients to active treatment status.
The authors note that for those patients with prostate cancers at low-risk for progression, the active surveillance strategy was proposed a decade ago but is recently receiving more attention as a viable management option.
"However, critical uncertainties still surround active surveillance," Pow-Sang said. "The criteria that qualify a patient as low or very low risk are not clear, nor is the definition of disease progression."
In this article, Pow-Sang and Buethe reviewed recent literature regarding those uncertainties, examining criteria for assisting in selecting men for active surveillance, including PSA density, percent free versus percent total PSA, biopsy positive core results and "volume of involvement" data.
"A PSA density of greater than 0.08 ng/mL/g has been identified as a significant predictor of future disease progression in those with low-risk prostate cancer," reported Pow-Sang. "However, some clinicians use a level of 0.15 ng/mL/g as a threshold."
Percent-free PSA has been found to be a significant predictor of organ-confined disease, said Pow-Sang and Buethe, noting that some reports have shown a significant indirect correlation between percent-free PSA and prostate cancer volume; but as yet this parameter is "not widely accepted as a predictor of cancer extent."
They also looked at studies on positive cores from biopsies and the extent to which core results could be used to develop criteria for treatment. Reports have shown the predictive value of positive biopsy cores, but the frequency for performing prostate biopsies remains controversial for outcomes of active surveillance patients, Pow-Sang said.
"Biopsies are not without consequences," he said. "Rectal bleeding and serious infectious complications from frequent biopsies need to be considered."
Additionally, no standard protocol exists, and controversy remains, for the frequency of PSA testing and the digital rectal exam for patients who are under active surveillance.
"Current guidelines vary," Pow-Sang said. "There are recommendations that the PSA and digital rectal exam should be performed every three to six months, or the PSA every three months and the digital rectal exam every six months. The frequency of biopsies is also variable, from once a year to up to every three years."
New potential tools
Pow-Sang and Buethe also report that a new tool, the PCA3 urinary marker, may be useful. PCA3 is a urine test used to quantify copies of the known prostate cancer gene 3. A PCA3 score of 35 or greater is considered possibly malignant. The test is prostate cancer specific and unaffected by prostatitis or benign prostatic hyperplasia, both of which have confounded the results of PSA testing.
The future of active surveillance
"Most studies report that 30 percent of active surveillance-classified men will be reclassified in the short term to clinically significant cancer that may require treatment," noted Pow-Sang. "Once more, PSA velocity (how quickly the PSA score rises) or PSA doubling time (how often the score doubles) has no clear role in active surveillance."
When counseling men with early prostate cancer, all treatment options, risks and complications should be discussed, Pow-Sang confirmed.
"Active surveillance is a reasonable management strategy for low-risk and very low risk prostate cancer, but allows for continual reassessment and identification of progressive tumors."

Passengers used belts to restrain a man who was taken into custody Tuesday night after allegedly harassing people ? including a woman who said he grabbed her hair and asked her to kiss him???aboard a flight from Chicago to Santa Ana, Calif.
Passenger Nona Pitts told NBC4 in Southern California that the man, identified by the FBI as 26-year-old actor-model Arash Durrani, sat across from her on United Airlines Flight 473. Pitts asked whether he was "on anything" because of his strange behavior.

"He was kind of slurring his words," Pitts said. "He just goes, 'Why, you want some?'"
"The stewardess came by and he said, 'I want another (Jack Daniels),'" she said. Pitts told the flight attendant she thought the man had enough to drink, but said he received "two more bottles of J.D."
Pitts, who owns an Orange County, Calif. salon, moved to a different seat, but Durrani followed her, she said.
"He grabs my hair and he goes, 'Nona, Nona, Nona, kiss me,'" said Pitts.
When the man started pacing and scaring children on the plane, passenger David Truong said he and other passengers took action and physically restrained the man with belts.
"He looked at every one of us and said, 'I'm going to kill you and your entire family,'" Truong said.
The man was restrained for nearly three hours.

The inquest into the death of Jacob Michael will begin on Monday 1 October at the Daresbury Park Hotel in Warrington.
Jacob Michael died on 22 August 2011 aged 25 following arrest and restraint by police.? He had called the police himself in an agitated state after telling his family he had been threatened.? The police arrived at the house and forced their way into his bedroom, spraying incapacitant spray at him, whereupon Mr Michael ran out of the house and down the street.? The police pursued him, striking him with batons and restraining him before putting him in the back of a police van to take him into custody at Runcorn police station.
He was then left face down on the floor of a police cell for several minutes with police officers standing on his legs, where he died.
On the first day of the inquest, the jury will be shown disturbing CCTV film of Jacob Michael being pursued by police officers wielding batons in the street outside his home, being transported in the police van, arriving at the custody suite and then being held face down on the cell floor where he died.? More than 60 witnesses are listed to give evidence including Jacob?s mother, civilian eye witnesses and police officers.? The inquest is scheduled to last for four weeks.
Jacob Michael?s family hope the inquest will answer serious questions concerning the officers? actions when they entered his bedroom, the use of force by police in the street, the restraint itself, the failure to treat him as a medical emergency rather than take him to the police station, his treatment in the police station, and the overall attitude of police to a young man who was clearly confused, frightened and unwell.

Acupuncture may go more mainstream now that new research shows that it provides modest but statistically significant benefits over both standard care and placebo for patients with chronic pain.
A team led by researchers at Memorial Sloan-Kettering Cancer Center in New York looked at 29 randomized controlled trials involving a total of 17,922 people from the U.S., Germany, Spain, and Sweden who suffered with back and neck pain, osteoarthritis, or chronic headaches. For greater precision, the investigators analyzed data on the individual patients, rather than outcome summaries, as prior reviews have done. The study was published online Sept. 10 by Archives of Internal Medicine.
In 18 of the studies, patients either got acupuncture, in which needles were inserted at points of the body traditionally used to provide pain relief, or standard treatments such as analgesics, physical therapy and exercise. In 11 others, acupuncture was compared with a control group that included "sham" acupuncture, in which needles didn't break the skin or were inserted superficially.

On our summer family vacation last month, we traveled through the east coast and I can?t recall a time when we crossed over more bridges. Some bridges were old, some were brand new, some low and some very high. Understanding how salamanders grow new limbs provides insights into the potential of human regenerative medicine
LA JOLLA, CA---- Based on two new studies by researchers at the Salk Institute for Biological Studies, regeneration of a new limb or organ in a human will be much more difficult than the mad scientist and supervillain, Dr. Curt Connors, made it seem in the Amazing Spider-man comics and films.
As those who saw the recent "The Amazing Spiderman" movie will know, Dr. Connors injected himself with a serum made from lizard DNA to successfully regrow his missing lower right arm - that is, before the formula transformed him into a reptilian humanoid.
But by studying a real lizard-like amphibian, which can regenerate missing limbs, the Salk researchers discovered that it isn't enough to activate genes that kick start the regenerative process. In fact, one of the first steps is to halt the activity of so-called jumping genes.
In research published August 23 in Development, Growth & Differentiation, and July 27 in Developmental Biology, the researchers show that in the Mexican axolotl, jumping genes have to be shackled or they might move around in the genomes of cells in the tissue destined to become a new limb, and disrupt the process of regeneration.
They found that two proteins, piwi-like 1 (PL1) and piwi-like 2 (PL2), perform the job of quieting down jumping genes in this immature tadpole-like form of a salamander, known as an axolotl - a creature whose name means water monster and who can regenerate everything from parts of its brain to eyes, spinal cord, and tail.
"What our work suggests is that jumping genes would be an issue in any situation where you wanted to turn on regeneration," says the studies' senior author, Tony Hunter, a professor in the Molecular and Cell Biology Laboratory and director of the Salk Institute Cancer Center.
"As complex as it already seems, it might seem a hopeless task to try to regenerate a limb or body part in humans, especially since we don't know if humans even have all the genes necessary for regeneration," says Hunter. "For this reason, it is important to understand how regeneration works at a molecular level in a vertebrate that can regenerate as a first step. What we learn may eventually lead to new methods for treating human conditions, such as wound healing and regeneration of simple tissues."
The research team, which included investigators from other universities around the country, sought to characterize the transcriptional fingerprint emerging from the early phase of axolotl regeneration. They specifically looked at the blastema, a structure that forms at a limb's stump.
There the scientists found transcriptional activation of some genes, usually found only in germlime cells, which indicated cellular reprogramming of differentiated cells into a germline state.
In the Development, Growth & Differentiation study, the research team, led by Wei Zhu, then a postdoctoral researcher in Hunter's laboratory, focused on one of these genes, the long interspersed nucleotide element-1 (LINE-1) retrotransposon.
LINE-1 elements are jumping genes that arose early in vertebrate evolution. They are pieces of DNA that copy themselves in two stages - first from DNA to RNA by transcription, and then from RNA to DNA by reverse transcription. These DNA copies can then insert themselves into the cell's genome at new positions.
A few years ago, Fred Gage, professor in the Laboratory of Genetics at the Salk Institute, discovered that LINE-1 elements move around during neuronal development, and may program the identities of individual neurons.
"Most of these copies appear to be 'junk' DNA, because they are defective and can never jump again," says Hunter. But all mammals, including humans, still have active LINE-1 genes, and the salamander, whose genome is 10 times larger than a human's, contains many more.
Active LINE-1 retrotransposons can keep jumping, and that was true in the developing blastema where LINE-1 jumping was dramatically switched on. But in the researchers' companion study, in Developmental Biology, they found that PL1 and PL2 switch off transcription of repeat elements, such as LINE-1. "The idea is that in the development of germ cells, you definitely don't want these things hopping around," says Hunter. "The mobilization of these jumping genes can introduce harmful genomic rearrangements or even abort the regeneration process."
In fact, when the researchers inhibited PL1 and PL2 activity in the axoloti limb blastema, regeneration was significantly slowed down.
"The need to switch on one set of genes to stop other genes from jumping just illustrates how amazingly difficult it would be to regenerate something as complex as a limb in humans," Hunter says. "But that doesn't mean we won't learn valuable lessons about how to treat degenerative diseases."

SAN FRANCISCO (Reuters) - Life-sciences and health investing, long shunned by venture-capital investors, is making a comeback.
Several venture firms globally have raised or are raising funds this year in the $200 million to $400 million range dedicated to life sciences, biotech, and medicine, while others have raised large, diversified funds with big chunks dedicated to those fields.
The fundraising builds off a rebound in life-sciences investing last year, when $367.6 million in venture money was invested in healthcare services, up 41 percent from 2010; $2.82 billion went to medical devices and equipment, up 17 percent from 2010; and $4.82 billion went into biotechnology, up 24 percent from 2010.
A handful of big acquisitions, new U.S. laws that could speed up drug approval in key areas, and a broader definition of what investors think belongs in those funds are helping to breathe life into the highly cyclical sector, investors say.
Health and biotechnology start-ups need every boost they can get.
Such companies rarely produce blockbuster initial public offerings. For biotechnology in particular, products have to clear high regulatory hurdles and often take over a decade to get to market. The failure rate is high.
Last year, Morgenthaler Ventures and Advanced Technology Ventures spun off their life-sciences investment teams and merged them into an independent group. Scale Venture Partners said it would stop investing in healthcare.
But the firms that remain in the field say they see hope.
Earlier this year Celgene Corp bought venture-backed drugmaker Avila Therapeutics for $350 million, a price tag that could rise to $925 million if Avila meets certain milestones.
Last year, Daiichi Sankyo bought drugmaker Plexxikon for $805 million, which could rise to $935 million if Plexxikon achieves certain milestones.
Big drugmakers see early-stage venture investing as a proxy for expensive in-house drug development. In part because the relative paucity of venture activity in biotech has raised concerns about the long-term product pipeline, they are increasingly investing directly alongside traditional venture investors.
Established companies "can invest in that innovation with a portion of the dollars they would have invested in their own R&D," said Marty Waters, a lawyer at Wilson Sonsini who helps arrange investments in the area.
Some have set up their own venture funds, as Merck & Co did last year when it created its $250 million Merck Research Venture Fund, joining several other major drugmakers with venture arms.
Those moves are in turn helping to encourage remaining venture-capital firms in the field. This year, the funds raised or being raised include Frazier Healthcare Ventures' $400 million fund, Longitude Ventures' $331 million fund, and a $200 million fund from Index Ventures, GlaxoSmithKline, and Johnson & Johnson.
In addition, generalist firms are raising funds with a heavy life-sciences focus, such as a $600 million fund from Canaan Partners, a $270 million fund from Flagship Ventures, and a $2.5 billion fund from New Enterprise Associates.
Partners say there is a sense that because so many firms have succumbed to the challenges and exited life sciences, the ones that remain will have a better shot at success.
Previously, "too much money was chasing too few deals," said Brent Ahrens of Canaan Partners. "If the too-much-money part goes away, that's likely to improve the ecosystem."
BIG IDEAS, NO GLITZ
Few expect life sciences overall will see the heady gains of the 1990s. And because the field doesn't produce glitzy companies, it will never attract the buzz that a sector such as the consumer Internet has enjoyed in recent years.
"This isn't Facebook or Zynga," said James Topper of Frazier. "Over the past ten years, biotech returns have been steadier than in tech. But you don't have a 100-times fund in biotech like you do in tech," he said, referring to a return of 100 times or more that a few funds have enjoyed as a result of giant scores on companies such as Google and Facebook.
But the different character of life-sciences start-ups is precisely the appeal for many investors. "Every therapeutic idea or product is a big idea," said Kevin Kinsella of Avalon Ventures, which just raised a $200 million life sciences and technology fund. "In technology, until proven otherwise, every idea is a small idea."
Meanwhile, lawmakers and regulators in Washington are working to speed up the approval process for drugs that could lead to breakthroughs in life-threatening diseases - and many investors hope that could in turn speed return on investment.
Some venture firms are also broadening the types of businesses they consider part of the life-sciences field, with ventures that apply information technology to healthcare problems of particular interest.
"There's been more of a consumer-Internet focus," said Annie Lamont of Oak Investment Partners, summing up the attitude as "let's take the consumer Internet and apply it to healthcare."
Oak backs companies such as Castlight, which allows patients to compare costs for medical procedures. The company recently closed a $100 million funding round and is considered an IPO prospect for 2013.
Other venture-backed companies that fit that mold include ZocDoc, which automates appointment booking; 23andMe, a company that allows customers to test and learn about their DNA; and medical-records company Practice Fusion.
Kleiner Perkins Caufield & Byers recently joined Mohr Davidow Ventures, Aberdare Ventures, and the Mayo Clinic as a partner in Rock Health, a program that seeks to foster start-ups that specialize in healthcare-related apps. Rock Health's first class of 35 start-ups graduated last month.
Even traditional healthcare services can sometimes produce venture-capital-style returns. When Universal Health Services acquired psychiatric-facility operator Ascend Health for $500 million in June, Polaris Venture Partners said it stood to earn $115 million, or ten times the firm's investment.
